Definition of Organization and Program Fields
Organization
Organization Name:
- The official name by which the organization is known. In some cases, the organization may be broken up due to its size/number of programs. In this case, the organization name may be the division name.
Organization AKA ("Also Known As"):
- The acronym, former name, or common name by which the organization is known.
Affiliated Organization:
- The group the organization is directly affiliated.
Organization Type:
- The organizational designation or status. The options available are: government, individual, non-profit, private or public.
Description:
- This is a narrative of the organization's main functions or purpose.
Programs
Program Name:
- The name by which the program is known. The collection of services may not have an official or formal name. In these cases, the name may be the same as the services it provides.
Program AKA ("Also Known As"):
- Any acronym or other name by which the program is known.
Program Description:
- This is a narrative of the program's goals or main function.
Announcements:
- A brief, timely statement about the program.
Partner Organization:
- Organization(s) that are directly associated with the program.
POPULATION SERVED
Target Population:
- The primary audience to be served by the program; not necessarily who the program serves by way of eligibility. The target population indicates that the program was designed to serve the needs of a particular group of people even if others would be eligible to participate. This field may be left blank when a service is open to all groups.
Age:
- Represents the age-range for whom the program will serve.
Language Available:
- Set of languages for which the program has the capacity to meet directly.
SERVICE(S)
Services Identified:
- The primary services or entry point services a person can receive without already being involved with the organization. The set of services selected here are chosen among a set of terms from the Alliance for Information & Referral (AIRS) taxonomy; a uniform structure for classifying resources in a database. Terms may be classified by: service activities; facility types; widely known programs; or delivery modalities.

INTAKE & ELIGIBILITY
Eligibility Requirements:
- The specific set of qualities that qualify an individual to participate in the program. These requirements may detail both inclusion and exclusion information.
Intake Process:
- The required process through which the program accepts participants. This may be through a registration process, telephone or walk-in, or only through a referral.
Referral Source:
- If a referral is identified as a required part of the referral process, the source is the individual or agency that must make the recommendation for the individual to participate in the program. They may be: the individual him or herself, a parent, the school, ACCESS Team, Child Welfare, Crisis Center, Any DHHS division, the police, Department of Juvenile Services (DJS), or other.
Program Fees:
- Payment that must be made to participate in the program. These fees may be on a sliding scale, waived or negotiable, but should be listed if expected of most participants.
Fee Description (Desc.):
- Description of the dollar amount listed in the payment field. Sometimes clarity is required noting whether the fee is per session, per month, or one-time only.
Payment Options:
- Alternatives available to fulfill the program fees required. These could include: fee-for-service; sliding scale which alters the amount based on the individual's income; by scholarship; third-party reimbursement, medial assistance, Medicare or other.
SERVICE AREA
Service Area:
- The geographic area the program serves. Service area is not the same as delivery location. Service area designates where a participant must reside or which school they must attend in order to qualify for the program. If it serves residents in all of Montgomery County, All of Montgomery County will be displayed.
- Region: refers to one of the five government service center regions designated for the regional delivery of public services.
- City/Jurisdiction: refers to the major cities, municipalities and jurisdictions in the county.
- School Cluster: refers to the county school cluster areas that generally include a high school and its feeder middle and elementary schools.
DELIVERY SITE
Delivery Site:
- The location where clients receive services. If the delivery site is the same as the organization's office location, the organization's address will appear. There may be more than one delivery site, serving the same population, offering the same service with the same intake and eligibility criteria as described above.
- Organizational Office: this site is the same location and address as that of the organization.
- Community Center: these sites include the county's community and neighborhood recreation centers.
- Community Health Center/Clinic: these sites are the DHHS public health centers and clinics.
- Regional Service Center: one of the five regional service center sites.
- School: Montgomery County Public Schools.
- Confidential Location: location that by nature of the service can not distribute specific address information.
- Home-based: the location is based out of the home of the individual being served.
- Community-based, not specified: location within the community that changes or depends on site availability
Capacity Number:
- The maximum or optimum number of participants for the program at the particular delivery site location. It is understood that for some programs this may be an estimate. Often if there is not an enrollment cap, there may be a standard professional-to-client ratio that can serve as a guide to capacity limits.
Snapshot Enrollment:
- The number of participants enrolled in the program at a specific point in time at the particular delivery site.
Snapshot Waiting List:
- The number of participants on a waiting list for the program at a specific point in time at the particular delivery site.
Hours of Operation:
- The days of the week and hours for which the program operates at the particular delivery site. Locations that have no specified hours can check 24/7 to designate operation 24hrs a day, 7 days a week.
- Variances: Text noting any changes in hours of service.
CONTACTS
Contact Types:
- The contacts represent individuals or phone numbers for a program. Contacts may only be an information or enrollment number.
