A unique, one-year program of services and support for young adults with disabilities who need, but have not found, a placement in an appropriate day program after aging out of educational entitlements. * Helps fill a pressing need in the community by providing services to individuals who need, but have not found, a placement in an appropriate day program after aging out of educational entitlements. * Offers an individualized program of activities and training based on each participant’s assessment and goals. * Serves as a portal for individuals and their families, introducing them to a broad range of community resources which will, in most cases, include an introduction to and collaboration with outside vocational or day habilitation programs. This collaboration will help individuals identify and become familiar with programs they might wish to enroll in, and allow outside service providers to likewise become more familiar with an individual’s capabilities.
